Confidentiality of Substance Use and Mental Health Records

Your records at Lifeline Recovery and Wellness may include sensitive information related to substance use or mental health treatment. These records are protected by federal law (42 U.S.C. 290dd–2 and 42 CFR Part 2). In general, we may not disclose this information without your written consent unless:

  • Required by court order

  • Necessary in a medical emergency

  • Used for research, audit, or program evaluation as permitted by law

  • Required for reporting certain crimes on premises or threats of harm

You may file a privacy complaint with us or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
